Whentheindexfingerisinjuredorseveredinconjunctionwithatraumaticamputationofthethumb,transferoftheinjuredindexfingercanrestoretheimportantfunctionofthethumb.Thepurposeofthisstudyistoevaluatetheresultsofthetransferofaninjuredindexfingerfortraumaticlossofthethumb.
Sevenpatientstreatedbypedicledtransferofatraumatizedindexfingerafteramputationtotheipsilateralthumbwerereviewedretrospectively.Postoperativeevaluationsincludedthumbrangeofmotion,oppositionandpinchfunction,graspandpinchstrength,sensation,apick-uptest,andapatient-ratedappearanceofthethumbandhand.Vascularpatencyofthetraumatizedindexfingerandthumbwasevaluatedineachpatientpriortothumbreconstruction.
Afteranaverageof4yearsoffollow-upforsurvivingpatients,allhadexcellentpostoperativefunctionandsatisfactoryresults.Theperiodbetweeninjuryandthumbreconstructionrangedfrom5monthsto4years.Allpatientsweremenwithameanageof43years.Amputationlevelsincludedthemetacarpophalangealjointin2patients,thefirstmetacarpalin2patients,andtheproximalphalanxin3patients.Alltransferredtraumaticindexfingerssurvivedwithoutcomplications.
Conclusions
Transferoftheinjuredindexfingertotheamputatedthumbservesasanexcellentadjunctfortreatmentoftraumaticthumbamputations/crushinjuries.Consistentresultscanbeobtainedwhilemaintainingoppositionandprotectivesensationafterthisprocedure.However,technicaldemandsaregreat,andinitialinjuriestothethumbandindexfingerultimatelydeterminethefinaloutcomes.
