【摘要】　　目的：比较颞肌外及颞肌下钛网修补额颞区颅骨缺损的临床疗效。方法：回顾性分析135例额颞区颅骨缺损的手术方法及疗效。结果：颞肌外钛网修补额颞区颅骨缺损72例；颞肌下钛网修补额颞区颅骨缺损63例，2组年龄、缺损原因、缺损时间、均无明显差异（p>0.05）；颅骨塑形不满意、皮下积液、脑表面出现点状出血等的发生率颞肌外修补较低，与颞肌下组有显著性差异。两组均未发生术后感染而取出钛网。结论：颞肌外修补法操作简单，手术时间短，术后修补区塑型满意、并发症少，手术效果优于颞肌下修补法。

　　1资料与方法

　　1.1一般资料

　　颞肌下钛网修补额颞区颅骨缺损手术组（以下称颞肌下组）63例，男39例，女24例；年龄13岁～65岁，年均35.5岁；缺损原因：脑外伤减压术后47例，高血压脑出血术后14例，粉碎性颅骨骨折碎片清除术后2例。颅骨缺损时间3个月～2a，平均3.8个月。颞肌外钛网修补额颞区颅骨缺损手术组（以下称颞肌外组）72例，男45例，女24例；年龄17岁～68岁，平均38岁；缺损原因：脑外伤减压术后54例，高血压脑出血术后15例，粉碎性颅骨骨折碎片清除术后3例。颅骨缺损时间3个月～6个月，平均3.5个月。
